Muse Family Practice is a family clinic in Trumann, AR, led by Keechia Muse, APRN. This page has two parts: how this website handles visitor information, and our HIPAA Notice of Privacy Practices for medical records.
This site is public information: hours, services, insurance, and how to call or walk in. We do not collect new-patient forms online. For medical questions, call (870) 650-4703 during business hours. Established patients can use the patient portal.
Website privacy
This site is a static public page. There is no contact form. If you email customercare@musefp.com or call (870) 650-4703, you choose what to share. We use that only to respond and to help you become a patient or schedule a visit. We do not use it for advertising, and we do not sell visitor information.
The host may log routine technical data (browser, device, approximate region, pages viewed, IP address, and cookies needed to load the site). We are not running a separate ad network.
The patient portal is Kareo, not this website. Portal messages and records follow Kareo’s terms and our HIPAA notice below. Please do not put sensitive medical details in email.
The website is meant for parents, caregivers, and adult visitors. We do not knowingly collect personal information from children through the website.

Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Effective date: August 13, 2026. This HIPAA notice applies to protected health information we create or receive about you as a patient — in the office, by phone, through the Kareo patient portal, and in your chart.
We are required to keep your health information private, to give you this notice, and to follow it. We will tell you if a breach of unsecured PHI happens that affects you. We do not sell your medical information. You may refuse to sign an acknowledgment that you received this notice. We can still treat you.
Treatment, payment, and operations. We use and share PHI to care for you, to bill you or your insurance, and to run the clinic (including our electronic health record). Vendors who help us may see PHI only to do their job, under a written agreement.
The law also allows or requires sharing in limited cases, including public health, health oversight, required-by-law reports, court orders, serious threats, and workers’ compensation. We share only what is reasonably needed, except for treatment and certain other cases the law treats differently.
Most other uses need your written authorization, including most marketing. You may revoke an authorization in writing, except to the extent we have already relied on it.
Your rights include inspecting or getting a copy of your record, asking for an amendment, asking for a restriction or confidential communications, getting a list of certain disclosures, and getting a paper copy of this notice. You may file a complaint with us by calling (870) 650-4703 or with the U.S. Department of Health and Human Services. We will not retaliate.
